REQUEST FORMS
Submit this form once you complete contracted service.
Request Time off/Schedule Change
Submit this form at least 72 hours in advance to request time off or a change to your contracted service . In case of an emergency (personal illness or family emergency) less than 72 hours in advance, contact PATH at 786.681.7284 AND submit this form.
Request Reimbursement for Approved Expenditures
Submit this form to receive reimbursement for pre-approved PATH related purchases. Please allow up to 14 days for approval.
STAFF PROTOCOL
APPROPRIATE DRESS
All site-based instructors should wear:
1. An approved PATH shirt (T-shirt or polo)
2. Either professional jeans or khakis
3. Clean shoes (sneakers or closed toe shoes)
STAFF MEETINGS
Please be prepared to participate in a production call each time you’re part of an event. Production calls and staff meetings will be coordinated by designated site coordinators.
